Wireless 5GHz 2/3 reboots is not active

I have a strange hardware problem with the 5GHz wifi card. I noticed that the 5GHz wifi is not functional, so I opened the device, as suggested in some other posts, and tried to move out the card and reseat it. After that the card worked, it was visible, but as soon as there is a firmware update, it does not initialise again. Basically 2 out of 3 reboots it does not work (shows “Generic 802.11bg - Device is not active”).

I do not know what to do, is there a way how to request a replacement card?
